Alignment Health Plan is an HMO, HMO-POS and an HMO SNP plan with a Medicare contract. Enrollment in Alignment Health Plan depends on contract renewal. The HMO-POS product will be available through Dec. 31, 2018. Medicare beneficiaries may also enroll in Alignment Health Plan through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
The Alignment Health Plan Heart and Diabetes (HMO SNP) plan has been approved by the National Committee for Quality Assurance (NCQA) to operate as a Special Needs Plan (SNP) until Dec. 31, 2021, based on a review of the Alignment Health Plan Heart and Diabetes (HMO SNP) plan’s Model of Care.
Out-of-network/noncontracted providers are under no obligation to treat Alignment Health Plan members, except in emergency situations. Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
Every year, Medicare evaluates plans based on a 5-star rating system.
