Special Needs Plan Training
Providers and staff responsible for managing Special Needs Plan (SNP) members must complete Alignment’s SNP and Model of Care (MOC) provider training within 60 days of contracting/employment and annually thereafter.

After the Alignment Health Plan Special Needs Plan Model of Care Provider Training has been distributed for the current coverage year, a designated representative should submit an electronic confirmation on behalf of Provider group by completing the online form. Individual Providers may also review the training and submit their own confirmation.
