    • C-SNP - Chronic Special Needs Plans (C-SNP) are designed for individuals with disabling chronic conditions. You must be eligible for Medicare Parts A and B and diagnosed with qualifying conditions such as cancer, dementia, HIV/AIDS, and autoimmune disorders.
